FORM OF DECLARATION FOR SURVIVING OFFICER OR SOLDIER

On this 21st day of June A.D., One Thousand Eight Hundred and fifty two, personally appeared before me, Daniel B ? a Justice of the Peace, duly authorized to administer oaths within and for the County and State aforesaid, Robert Baker aged 78 years, a resident of Clay County in the State of Kentucky who being duly sworn according to law declares, that he is the identical Robert Baker who was a Liutenant in the Company commanded by Captain Thos. McJilton in the 11th Regiment of volunters commanded by Co Mc Williams in the War with Great Britten North western Army. that he volunteered at Manchester Ky on or about the 10th day of August A.D. 1813 for the term of during the war and continued in acrual service in said war, for the term of ??months and was honorably discharged at or near ? on the opposite bank of ? on the 20th day of November A.D., 1815 as will appear by the mustor rolls of said company having long been lost his certificate of discharge. ??????????????????????????????????????????????????????????

POWER OF ATTORNEY

Know all men by these presents, That I, Robert Baker (his signature) of the County of Clay and state of Kentucky have constituted and appointed, and by these presents, do constitute and appoint GEO. H. MONSARRAT my true and lawful attorney, irrevocably for me and in my name, to receive from the United States the Bounty Land Certificate or Warrant, or Pension Certificate, and all such sum or sums of money due me on account of arrears of pay, three months extra pay, forage, mileage, for horses and other property lost or destroyed in the military service, expenses incurred or money expended for organizing volunteer companies before being mustered into the service of the United States, or any money or moneys which may be due me by the United states on any account whatsoever, and generally to do all lawful acts and things whatsoever touching the premises and an attorney or attornies under him for the purposes.

On this 8th day of August 1855 before me, a Justice of the Peace in and for the County and State aforesaid personally appeared Robert Baker aged eighty one years the 1st March last, who being duly sworn according to law, declares that he is the identical Robert Baker who was a Lieutenant in the Company Commanded by Captain Thomas McJilton in the 11th Regiment Kentucky Volunteers commanded by Col. William Williams. That he volunteered at Manchester, Clay County in the State of Kentucky on or about the 15th day of August 1813 was mustered out of service at a place in the State of Ohio opposite Maysville, Ky on or about the (20th) day of November 1813, and honorably discharged at said time & place or in a few days after and on his arrival home in Clay County, Kentucky.

He further states that he is the identical Robert Baker who was a private in the company Commanded by Captain Jacob Tipton in the (2nd) Regiment Commanded by Col. (William) Darke. That he enlisted at Jonesborough in what is now the State of Tennessee on or about the 1st day of May 1791, was in St. Clairs defeat and was engaged six months in service in said Regiment.

He further states that he has received from the service aforesaid in 1813 Bounty Land Warrant No. (not known) of forty acres, issued and in the Act of Congress .... by reason of said service which he has since legally disposed of and cannot now return; that he has received for the service aforesaid in 1791, Bounty Land Warrant ... (not known) of eighty acres, issued under the Act of Congress by reason of said service which he has since legally disposed of and cannot now return.

He makes this declaration for the purpose of obtaining the additional Bounty Land to which he is entitled under the Act of March 3rd 1855, never having received Bounty Land except as above stated, and he hereby constitutes and appoints Charles C. Ticher(?) of Washington, D. C., as his attorney to prosecute his claim and procure his warrant, thereby authorizing and empowering his said attorney to obtain from the proper authorities at Washington City or elsewhere, certified copies of any Army Records or documents, in which his said services may be recorded, and to procure any and all other evidence that may be necessary to establish his claim, as fully as he might or could do if personally present, and revoking & countermanding all other authority that may have been given for the above specified purpose.
